      MAV Supervisors: Complete this form in order for MAV Gear to be prepped and ready for your next deployment.
      SUPERVISORS: Please complete this form with the MAV Gear you need for your upcoming event. A separate form must be completed for individual events and must be submitted at a minimum of 72 hours prior to the start of the event. All gear will be boxed and ready for deployment. Gear will be available for pickup the night before the event after 5 p.m.
